Forms

Members, please remember to submit your Member Activity Logs (form 7029) to your Information Systems Officer monthly.  The system will not accept anything older than 90 days.

There are three basic ways of submitting forms.  The two electronic options are by far the fastest, cheapest and easiest to maintain records.  Here they are in order of preference.

  1. If you have full Adobe Acrobat or Adobe Acrobat Approval, you may fill out the form, save it to disc and e-mail it to FSO-IS@Flotilla2-3.org.  Remember, the free Adobe Acrobat Reader will not allow you to save to disc.
     
  2. Acrobat Approval is no longer sold and does not work with newer versions of Acrobat.  A full version of Acrobat 7 can be purchased here for about $175.
     
  3. If you don't have either of these programs, you can still submit the most commonly used forms electronically via EMAIL FORMS using the free Adobe Acrobat Reader Get Adobe Reader

    This site allows you  to enter the data, print a nicely formatted version for your records, and send a condensed electronic copy to your FSO-IS.
     
  4. The third method for those who don't have e-mail is to print a blank form (or obtain one at your next flotilla meeting) and fill it out manually.  The form may be mailed to your Flotilla Information Systems Officer at the address listed in your Members Directory.
